Formaly

Formaly lets users describe what they want to learn, and AI generates a conversational form in seconds. Respondents complete forms through a chat-style experience rather than static questions. The page states that forms can be imported from Google Forms, Typeform, or Tally and rebuilt as conversational Formaly surveys. It also states that AI extracts themes, sentiment, and key quotes from responses. Forms can be shared via public link or embedded, and respondents can answer without an account. The site lists plans with credit-based usage, and mentions a public API and embeddable widget.
